Applicants must pay a $50 non-refundable application fee. You can phone or fax in your credit card number or send a cheque or money order. Please do not send cash.
In addition to completing the online form, these are the remaining required forms to complete your application to CHS. You can download the following application forms to your computer, print them, and give them to your references to fill out and send back to us. Reference forms must be signed and dated by both you and the person filling out the reference form. All forms are in Adobe Acrobat PDF format. If you do not have the Acrobat Reader, you can get it for free at the Adobe Website.
